Attorney Requests:  Click on the Attorney's Office Record Request Instructions for information regarding your requests.

Individuals:  You will need to call 859-238-1133 to schedule a time to pick up a copy of your      records.  The patient will need to be present to pick up a Medical Record.  Records for Minors can only be picked up by a parent or legal guardian.  If you call ahead we will make every effort to have the records ready for you when you arrive.

You will need to bring the following items with you: 

1.  The date of the transport.

2.  Your driver's license or some other form of picture ID with your name and address.
